our services

The difference is patient-centered care.

From receptionist to director, every one of our highly-skilled and well-trained staff set standards for themselves that inspire them to exceed what's expected. We hire people not only for their clinical skills, but for their capacity to care. Providing the best in facilities also leads to low staff turnover, so patients benefit from seasoned professionals who work together as a compassionate, interdisciplinary team.

A JCAHO Accredited facility, Baypointe offers 169 beds for skilled nursing, long term care, and rehabilitation, including Cedarwood, a 41-bed short term rehabilitation unit. Our multidisciplinary approach includes caring for patients’ medical, therapeutic, social and nutritional needs.

We provide:

  • Extensive, short-term rehabilitation.
  • Supportive, long-term care with restorative therapies.
  • Skilled nursing and medical care.

At Baypointe, we accept patients seven days a week, around the clock for your convenience, reservations are available for post-operative stays. A unique service is our Admissions Nurse, whose sole function is to ensure your admission goes as smoothly as possible. She will communicate with your doctor and/or hospital, explain what you can expect, get answers about lab results and ensure you understand the therapy ordered for you. With one-on-one attention, we can ease your transition and calm any fears.

The one on one attention continues upon discharge. When you’re ready, our RN Case Manager leads an interdisciplinary team in developing a plan of care in conjunction with care available in the community, such as visiting nurses.

A complete continuum of care.
Baypointe patients have the advantage of priority access to Heights Crossing, our sister assisted living facility right next door. It's extra peace of mind knowing help is available as you transition through recovery. In addition, being on the same campus is especially helpful for couples with differing care needs.
